📤 Admissions Submission Instructions
Before you begin:
Please download the admission forms above, fill them out completely, and save each one as a PDF before submitting.
🧩 Step-by-Step Instructions
- Download all forms from the section above on this page.
- Fill out each form digitally using Adobe Acrobat Reader (preferred) or print and complete by hand.
- Save each file clearly labeled, for example: StudentName_Application.pdf.
- Submit your completed forms using one of the two options below:
- Option 1 – Upload Online: Go to the Admissions Upload Center and upload each PDF file (Application, Health Record, etc.).
- Option 2 – Print & Deliver: Bring your signed printed forms to the school office in person.
Accepted formats: PDF (preferred) Max file size: 20 MB per file
Note: Uploading via Google Forms requires a Google account. If you experience issues, please email your documents to info@liceoaguadillanopr.com or bring them in person to the office.
